Thermage Results: Judging After Week 1 Means Missing the 6-Month Payoff
Why You Should Never Judge Thermage Results at Week 1 — Real Cases That Explain Everything

Thermage Results: Judging After Week 1 Means Missing the 6-Month Payoff
Last Tuesday, a woman in her mid-40s came into my clinic.
She had received Thermage FLX 600 shots at a Gangnam clinic,
and after two weeks of seeing "zero change," she had even reached out to ask for a refund.
She told me she'd looked up reviews online and found that about half of them said the same thing — "no effect" —
and she wanted to know if this was just a scam procedure.

You probably know what Thermage is — so why do the reviews vary so wildly?
Thermage uses Radiofrequency (RF) energy to heat the Dermis, stimulating Collagen contraction and regeneration for a Lifting effect.
Unlike Ultherapy, which uses ultrasound to directly target the SMAS Layer,
Thermage heats the dermis broadly across the entire surface to restore the skin's own Elasticity.
That's why it tends to work better for skin laxity and loss of firmness rather than significant sagging.
Why do Week 1 reviews and Month 6 reviews say completely opposite things?
The key point of this post
About half of the "no results" Thermage reviews are written within the first week after the procedure.
Collagen remodeling doesn't begin in earnest until around the 3-month mark,
and peaks at 6 months. Judging too early means missing the window where results actually show.
This might be hard to grasp as just words on a page, so let me walk you through a real case.
Let's go back to that 44-year-old patient I mentioned.
She came in two weeks after receiving 600 shots elsewhere, saying she'd seen "zero effect."
What I did that day wasn't another procedure — it was an explanation.
To be honest with you, Thermage produces very little noticeable change in the first day to one week after the procedure.
The slight firmness some people feel during that window is closer to temporary Swelling from the heat than actual results.
The real outcome appears as new Collagen is formed and undergoes remodeling —
a process that begins in earnest around the 3-month mark and peaks at 6 months.
So judging Thermage as "ineffective" at week one is a bit like opening the oven the moment you put the dough in and saying it hasn't risen.

In her case, I pulled up her before-and-after photos for a side-by-side comparison.
When we placed her Week 2 photo — the one she thought showed "no change" — next to her pre-procedure photo,
there was a subtle but visible improvement along her jawline definition.
It just wasn't the dramatic "lifted" feeling she had been expecting.
That part comes at the 3-month mark.

Dr. Wi Young-jin's Key Takeaway
Thermage is not a procedure that delivers an instant "pulled tight" result within a week.
Collagen remodeling begins at the 3-month mark and peaks at 6 months.
If you evaluate based on Week 1 reviews, you'll miss the entire window where real results appear.
The Downtime factors that determine satisfaction vs. disappointment in Thermage reviews
When you look at it this way, the difference becomes clear.
Based on statistics from my own clinic,
about 30% of patients who received Thermage last year reported satisfaction at the 1-month mark.
But when I followed up with those same patients at 6 months, satisfaction had risen to over 70%.
The procedure itself hadn't changed — only the timing of the evaluation had.
Another case that comes to mind is a 55-year-old patient
who came in after having the procedure elsewhere, saying "my jawline looks more defined, but my face feels like it lost volume."
This is one of the most common misconceptions I see in reviews.
Thermage is not a procedure designed to reduce fat —
but in patients with a slimmer face or less facial volume, the Swelling from the heat can subside and temporarily leave the face looking slightly hollowed.
That said, this isn't necessarily a drawback.
By the 6-month mark, as Collagen fills back in, the face settles and looks fuller again.
The key is assessing your facial type from the start and customizing the treatment area and number of shots accordingly to minimize this from happening.

3 Questions I Get Asked Most Before a Thermage Procedure
Q1. When can I return to daily life? Can I go to work the same day?
A. Well,
this answer takes a moment to unpack —
the procedure itself allows for makeup application on the day of, and it typically won't interfere with going to work the next day.
Bruising or Scabbing is rarely an issue.
That said, you may notice some mild redness or a lingering Heat sensation for about 2–3 days after the procedure.
With a mask on, it's generally not noticeable.
I usually recommend a Friday procedure → rest over the weekend → back to work Monday pattern,
but if your schedule doesn't allow for that, an evening procedure on a weekday followed by work the next morning is also perfectly fine.
Once patients hear that, they often have one more question:
Q2. How long do results last? Will I need to come back after a year?
A. Well,
this one also requires a bit of explanation —
on average, results tend to last around 12 to 18 months.
That said, there's quite a bit of individual variation.
Depending on skin thickness,
sun exposure habits,
weight fluctuations,
and facial muscle patterns, some patients see results hold for as little as 10 months,
while others maintain them for close to 2 years.
What I typically recommend is
evaluating your own photos at the 6-month mark,
then planning a touch-up procedure somewhere between 1 and 1.5 years based on those results.
It's not a procedure you need to repeat every year.
One more thing before I wrap up:
Q3. What are the potential side effects or precautions I should know about?
A. Honestly, in my clinic, the most common "side effect" I see is a mismatch in expectations.
Medical side effects are uncommon.
In rare cases, temporary Erythema,
mild Swelling,
and very rarely, superficial burns may occur —
but when the procedure is performed at the correct energy output and depth, issues are rare.
The real precautions are a bit different.
Patients with a lean face and less facial fat,
or those with prominent cheekbones, may experience the appearance of slightly hollowed cheeks if the treatment area is not selected carefully.
Also, for 2 weeks after the procedure, please avoid saunas, jjimjilbangs (Korean sauna rooms), and excessive alcohol.
Consistent Sunscreen use is also more important than usual during this period, as it supports proper Collagen regeneration.
If you take one thing away from today's post — Thermage reviews are meaningless without context about timing. A dissatisfied review at Week 1 and a satisfied review at Month 6 are simply two different points along the same procedure's timeline.
In my next post, I'll cover "How to self-evaluate your Thermage results at the 3-month mark — photo angles, lighting, and comparison tips." I'll show you, through real cases, how to tell whether you're genuinely seeing results or whether the procedure truly didn't deliver. This has been Dr. Wi Young-jin.
