After lip filler, gentle face washing is usually fine the same day, strenuous exercise can wait until the next day, and you should avoid touching or pressing the injection sites for about three days. For lipstick and alcohol, there's no fixed official date, so it's safer to decide based on how the needle marks and swelling settle. Swelling and bruising are common and expected. If your lips turn pale or the pain keeps getting worse, though, contact a doctor right away.

In this article, we'll walk through how long swelling usually lasts, which reactions need care immediately, what to do from the day of treatment through week two, and when you can actually judge whether the result looks natural.

The short version

  • Swelling and bruising are common and usually settle within 1 to 2 weeks. Judge how natural your lips look after that.
  • Exercise from the next day, hands off for about 3 days: on the day itself, stick to cold compresses and gentle washing.
  • There's no official date for alcohol or lip makeup, so set it with your provider.
  • If your lips or nearby skin turn white or blue, or the pain keeps getting worse, get medical care right away.

How Long Do Swelling and Bruising Last After Lip Filler?

Lips have thin tissue and plenty of blood vessels, so swelling and bruising show up easily after filler. If your lips look fuller than expected the next morning, most of that is swelling rather than the amount of filler. That's why it's better not to decide you got "too much" based on the first two days.

A meta-analysis of 16 randomized lip filler trials found that half of participants (50%) had some reaction after treatment. The most common were swelling (78%), firmness (48%), bruising (34%), and tenderness (33%). Serious reactions, meaning ones that needed treatment or lasted beyond 30 days, occurred in 1.1%. In other words, reactions are common, but severe ones are rare.

In a separate trial of 180 patients, swelling appeared in 58% and bruising in 44%, and 88% of these reactions were mild. Redness, swelling, tenderness, and bruising typically clear within 7 to 14 days.

Side Effects: What's Normal and What Needs Care Now

Normal swelling and bruising get a little better each day. If something is getting worse over time, changing color, or becoming more painful, it isn't following the usual course.

Usually passes on its own

  • Plump, swollen lips that look puffier in the morning (a few days to 2 weeks)
  • Small bruises at the injection points and a tender feeling when pressed
  • A slightly firm or lumpy feel when you touch the lips

Contact a doctor right away

  • Lips or nearby skin turn white, gray, or blue, or the pain keeps getting worse → get medical care immediately
  • Blurry or changed vision → go to the nearest emergency room
  • Slurred speech, numbness or weakness in the face, arms, or legs, trouble walking, severe headache, or dizziness → go to the nearest emergency room

The U.S. FDA warns that filler injected into a blood vessel can lead to skin necrosis, vision problems including blindness, and stroke. These signs can appear during or after treatment. Don't wait for a message reply; get care first. Swelling that grows again along with warmth also needs a doctor's check.

Day 1 to Week 2: What You Can Do and What to Hold Off On

Right after treatment, the focus is on leaving your lips alone and keeping them cool. The American Academy of Dermatology notes that your provider may suggest icing for 15 to 20 minutes before you leave, waiting until the next day to exercise or do anything strenuous, and avoiding touching the treated area for three days. Stay out of the sun and skip tanning, too.

When you wash your face, rinse gently with water instead of wiping your lips with a towel. For the first few days, it also helps to cut back on sucking hard through a straw or biting your lips.

Makeup, Exercise, and Alcohol: When Is Each One OK?

Face makeup can go on the same day. Products you apply directly over the needle marks, like lipstick or gloss, are a different story: check that the marks have closed, and start when your provider says it's OK. If you really need lip balm, use clean fingers or a fresh product and apply a thin layer without rubbing.

Start exercise gently from the next day, and ease off if the swelling comes back. As for alcohol, the official guidance we checked doesn't give a date, so we won't make one up here. If you have plans coming up, tell your provider the date before treatment and decide together.

At Beautystone, a clinic in Seoul's Hapjeong area, recovery consultations start by separating normal reactions from warning signs, and only then plan your daily schedule around them. If you take blood thinners or supplements that affect bleeding, mention it beforehand, since they can make bruising more likely.

When Can You Tell If Your Lips Look Natural?

Judge how natural your lips look 1 to 2 weeks later, once the swelling has gone down. Compared with photos from day one, almost any lip filler looks overdone. Take front and side photos in the same light before treatment, then retake them about two weeks later under the same conditions.

If your lips are very thin, trying to make them much fuller in one session can look unnatural. In the 180-patient trial, each lip received 1.5 mL or less, and 93% still gained at least one grade of fullness at 8 weeks. Starting small and reassessing after the swelling settles is a sensible order.

The Bottom Line: Plan Your Dates Before You Book

Swelling and bruising are expected and usually fade within two weeks; color change or worsening pain is not, and needs care right away. Exercise can resume the next day, the injection sites should stay untouched for about three days, and alcohol and lip makeup are best timed with your provider.

If you have an event or photo shoot coming up, leave a buffer of 1 to 2 weeks. When you reach out through the consult button on this page, include front and side photos of your lips, the shape you have in mind, and any past lip filler with product names and dates.

Frequently Asked Questions

Q. How long does lip filler swelling last?

A. According to the American Academy of Dermatology, swelling, bruising, and redness usually clear within 7 to 14 days. The first two days tend to be the puffiest, then it eases. If swelling gets worse over time or comes with warmth or increasing pain, contact your provider right away.

Q. Can I wash my face after lip filler?

A. Yes, gentle washing is fine the same day. Rinse with water and avoid rubbing or pressing your lips with a towel. Since the injection area should stay untouched for about three days, put off cleansing devices and facial massage until after that.

Q. When can I wear lipstick or lip balm?

A. There's no fixed official date. Because these go directly over the needle marks, check that the marks have closed and start when your provider says it's OK. If you need lip balm sooner, use clean fingers or a fresh product and apply a thin layer without rubbing.

Q. When can I work out after lip filler?

A. The American Academy of Dermatology advises waiting until the next day for exercise or other strenuous activity. Start gently, and ease off for a few days if your lips swell again afterward. Hold off on sports where your face could get hit until the swelling has settled.

Q. When can I drink alcohol after lip filler?

A. The official guidance we checked doesn't give a date for alcohol, so we won't put a number on it. If you have plans in the first 1 to 2 weeks while swelling and bruising settle, mention the date before treatment and decide with your provider.

Q. Is bruising on my lips normal?

A. In a trial of 180 patients, bruising appeared in 44%, and most cases were mild. A bruise that turns yellow and fades is following the normal course. Skin that turns white, gray, or blue with worsening pain is different and needs medical care right away.

Q. One side of my lips looks bigger. Is it uneven?

A. Swelling rarely goes down evenly, so one side often looks fuller for the first few days. That's why shape is judged 1 to 2 weeks later. If a difference remains then, take front and side photos in the same light and check in with your provider.

Q. How long does lip filler last?

A. The American Academy of Dermatology lists hyaluronic acid fillers as lasting about 4 to 12 months. Lips move constantly when you talk and eat, so duration varies by product and person. Ask how long your product usually lasts in the lips so you can plan your next visit.

Q. What should I tell my provider before lip filler?

A. Tell them if you're pregnant or breastfeeding, what medications and supplements you take, any allergies, any history of cold sores around the mouth, and past lip filler with dates. Blood thinners can increase bruising. Sharing upcoming event dates also helps with timing.
