"Doctor, How Many Sessions Does a Butt Filler Take?" — A Question I Got Last Week
Here's how many butt filler sessions are actually appropriate — explained through a Sculptra combination protocol

"Doctor, How Many Sessions Does a Butt Filler Take?" — A Question I Got Last Week
Last Thursday,
a woman in her late 30s came into my clinic.
She told me her tailbone was digging into the seat whenever she sat down,
and that her hip dips had gotten deeper — to the point where her pants just wouldn't fit right anymore.
"Doctor, one session of butt Filler should do it, right?"
— I hear this question
honestly three or four times a week.
But here's the thing —
it's not a question with a one-line answer.
Butt Filler: The Trap Behind "Done in One Session"
Butt Filler is a procedure where
high-viscosity Filler is injected into areas with hip dips, tailbone hollowing, or volume loss
to restore shape and contour.
Unlike facial Filler, the buttocks require
a significantly larger volume of product.
We're talking 20–40cc per side,
which means at least 50cc total for both sides.
So when people say "done in one session,"
that really only applies to patients who need very minimal volume.
For most people, that's simply not the case.
How Many Sessions of Butt Filler You Need — This Is Where It Varies
Key Takeaway
How many butt filler sessions are appropriate — Dr. Wi walks you through the Sculptra combination protocol
Butt Filler is about "adding volume,"
while Sculptra is about "building tissue firmness from within."
Because they serve different roles,
combining both is actually the most efficient way to reduce the total number of sessions.
Let me share a case I saw last month.
A patient came to me after having butt Filler done twice at another clinic —
she said the shape kept collapsing shortly after each session.
Here's why that happens:
when the subcutaneous tissue in the buttocks is thin and already sagging,
repeatedly injecting Filler alone causes it to shift downward under gravity.
It's essentially like stacking weight on top
without any structural support underneath.
So this is the protocol I typically recommend:
One session of Sculptra first,
followed by one session of butt Filler 4–6 weeks later,
and if needed, one reinforcement session 3 months after that.
Sculptra contains PLLA (poly-L-lactic acid),
a bioabsorbable ingredient that,
once injected, gradually stimulates your own Collagen production from within.
With 2–3 sessions spaced 4–6 weeks apart,
the tissue itself becomes noticeably firmer.
When you lay that foundation first,
the Filler placed afterward
holds its shape significantly longer.
That's exactly why patients who previously only had Filler elsewhere
tell us after 2–3 combination sessions at our clinic,
"It finally stays in place."
When someone asks how many sessions of butt Filler they need,
my first response is always: "Let's assess the tissue condition first."
If only a small amount of volume is needed, one session may be enough.
If sagging is also present, a Sculptra combination over 2–3 sessions is the right approach.
Getting that call right makes a substantial difference in how long the results last.
How Many Sessions of Butt Filler — My Recommendations by Patient Type
In the clinic, I typically break it down into
three patient profiles.
That said, this approach isn't without its trade-offs.
There's one thing I always make sure to mention:
combining Sculptra takes more time
and comes with a higher upfront cost.
However, when you factor in 1–2 years of longevity,
the total cost tends to be comparable to — or even less than —
repeatedly getting Filler alone.
That's why, for patients with noticeable sagging,
I still recommend this combination approach first.
Butt Filler: Three Things to Check Before Your Procedure
Q1. If I only get one session of butt Filler, how long will the results last?
A. It depends on the type of Filler used and the volume injected,
but high-viscosity hyaluronic acid-based Fillers
typically last around 12–18 months.
That said, if you sit for long hours
or experience significant weight fluctuations, results may fade more quickly.
Q2. Can I get Sculptra and Filler on the same day?
A. I generally don't recommend it.
Allowing Sculptra to firm up the tissue first,
then adding Filler 4–6 weeks later,
produces a noticeably better-defined result.
If you rush and do both on the same day,
the outcome can end up looking less refined than expected.
Q3. How long do side effects or Bruising last?
A. Because a larger volume is injected,
Bruising can persist for about 1–2 weeks.
In rare cases, nodules or asymmetry may occur —
these are largely dependent on injection depth and distribution technique,
which is why the provider's experience really matters.
For the first 2 weeks, please avoid saunas and intense exercise.
