Masseter Botox: Is It Bone or Muscle? How to Tell in 3 Seconds
Masseter Botox is most effective for those with muscle hypertrophy — where the jaw visibly bulges when clenching. Learn how to distinguish between a bone-type and muscle-type jaw, and understand how the procedure works.

Q. If I have a square jaw, can't I just get Botox and be done with it?
A. Not quite.
Whether your square jaw is caused by bone or muscle
makes a significant difference in results.
Botox only works on muscle — not bone.
Q. How do I know if my jaw is muscle-type or bone-type?
A. Clench your teeth firmly and check whether
the sides of your jaw visibly bulge outward.
Go ahead and feel that area with your fingers.
📌 Key Takeaways from This Post
Masseter Botox is most effective for those
whose jaw muscles protrude when clenching their teeth.
It slims the lower face by targeting
muscle hypertrophy — not bone structure.

Clench your teeth right now.
That's where your answer lies.
When people start researching square jaw concerns,
they're quickly flooded with information —
'how many units,' 'which clinic is cheaper,' 'how long does it last.'
But before any of that,
there's one thing you need to check first.
Clench your teeth firmly, right now.
Does the muscle along your lower cheek and jaw
visibly bulge outward?
When you press on it with your fingers,
does it feel firm and tense?
If so, you have masseter muscle hypertrophy.
The masseter is the primary chewing muscle,
and when it becomes overdeveloped, the lower face appears wider.
Botox works by being injected directly into this masseter muscle,
reducing its activity over time.
As the muscle becomes less engaged, it gradually atrophies and thins,
which results in a slimmer, more contoured lower face.
On the other hand, if you clench your teeth and notice little to no change?
That may indicate that the width comes from the bone structure itself.
To be straightforward — for those patients,
Botox is unlikely to produce the dramatic results they're hoping for.
Bone cannot be reduced with Botox.
This is a common misconception worth addressing:
masseter Botox doesn't eliminate a square jaw entirely —
it specifically reduces a square jaw caused by muscle overdevelopment.
Identifying which type you are is the essential first step.
👨⚕️ Dr. Wi Young-jin's Key Summary
If your jaw muscles visibly protrude when you clench your teeth,
you have masseter muscle hypertrophy.
Botox targets this muscle directly
to create a slimmer facial contour.
If the cause is bone structure, however,
the effects of Botox will be limited.
Understanding your jaw type before the procedure
is what ultimately determines your results.

If you're the muscle type,
here's how to approach it
Once you've confirmed you're dealing with muscle hypertrophy,
the next consideration is the thickness and distribution of your masseter.
Some patients have a masseter that spreads broadly across the jaw,
while others have a single focal point of thickness.
Every case is different, and here's how I approach it:
During your consultation, I personally assess the masseter by palpation,
and determine the precise injection sites and dosage
on the spot, tailored to your anatomy.
The reason this matters is that
you simply can't follow a fixed unit count.
If too little is used for a large, thick masseter,
the effect will be weak and short-lived.
Conversely, using too much on a thinner masseter
can excessively reduce chewing strength and cause discomfort.
There's also an important expectation to set here.
Many patients expect dramatic results immediately after the procedure,
but masseter Botox works gradually
as the muscle slowly atrophies over time.
Most patients begin to notice a change around weeks 2 to 4,
with the full, optimal results best assessed
at 1 to 2 months post-treatment.
One more point I always make sure to communicate:
the results are not permanent.
Individual variation exists, but typically after 4 to 6 months,
the muscle gradually reactivates and the effects diminish.
That said, patients who maintain consistent treatment over time
tend to see the masseter progressively thin with each session,
which means the intervals between treatments often extend.
We've also seen patients who reported no results elsewhere
achieve satisfying outcomes after 2 to 3 sessions with us —
in most cases, the initial dosage used elsewhere was simply too low.
The right dosage matched to your muscle size
is what delivers real, visible results.
Frequently Asked Questions
Q1. Will Botox make it hard to chew after the procedure?
A. This is one of the most common questions I receive.
In everyday meals,
most patients experience little to no discomfort.
However, very tough or hard foods — such as chewy meats
or hard nuts — may feel slightly more challenging
in the first few weeks after treatment.
Since the masseter is directly involved in chewing,
a temporary reduction in muscle activity does mean some reduction in bite force.
This typically resolves as you adjust naturally over time,
and it rarely interferes with regular, everyday eating.
Q2. If my face becomes too slim, will I look older?
A. Honestly, this concern isn't entirely unfounded.
Excessive atrophy of the masseter
can cause the face to appear hollowed or gaunt.
This is particularly relevant for patients in their 40s and beyond,
when natural loss of facial fat volume is already occurring —
reducing the masseter on top of that
can sometimes create an unexpectedly drawn or aged appearance.
For that reason, I generally recommend a gradual, staged approach
for patients in their 40s and older,
or for anyone who already has noticeable volume loss in the cheeks,
rather than going straight to the maximum dose.
